Three Steps to Determine Paternity for Legal Purposes
1. Order online or call (480) 292-9820 to order your Complete Legal Paternity Test. Appointments are scheduled three to four days in advance. The $30 charge per person for your father and child's appointments is included in the price of 455.00, but if you wish the mother tested as well, there is an additional 30.00 charge for her. Add the mother by ordering the Legal Paternity Added Specimen Collection Fee. If individuals are located in different areas, DNA Consulting will set up collection appointments at multiple locations. Once you are satisfied with the scheduled appointments and locations DNA Consulting sends the collection kit, with all the forms and instructions, directly to the specimen collection facility.
2. Follow the easy instructions to have the specimens collected at the facility.
For added convenience, DNA Testing Systems pays for all specimen collection fees. You will NOT pay anything to the collection facility. The alleged father's and child's specimens are required to determine paternity. If the mother is available, it is highly recommended that she submit her specimens for testing (at an additional cost of 30.00). Including specimens from the mother can greatly increase both the 'power of discrimination' and the 'probability of paternity'. Fingerprints and pictures may be taken at the time of collection to ensure proper identification of subjects in the paternity analysis. It is a good idea, but not necessary, to bring a recent photograph of the child to the collection facility if a picture ID or birth certificate is not available.
At the Specimen Collection Appointment, Parents and Participants are required to bring:
Valid Picture ID (adults)
Birth Certificate (children)
Verification of informed consent from each test participant (Many states require the signature of the biological mother for children under 18 years of age).
NOTE: If either of these forms of ID are not available additional paperwork will need to be completed.
3. After collection the specimens will be sent to our laboratory in Salt Lake City, Utah, for DNA extraction and paternity testing. Initial results will be provided via email or telephone, with a sealed hard copy of the document mailed to the person ordering the test. Express courier service can also be arranged.
Attention New York State Residents: New York State Law requires that all DNA paternity tests including a participant in the State of New York must include a Doctor's prescription (or a court order) and a documented chain-of-custody for all the specimens. Personal and/or anonymous DNA testing is prohibited in the State of New York. DNA Testing Systems does not provide DNA testing for New York State residents.
Add the mother for an extra $30.00.
